Why am I getting a Popeye arm? A tear of the long head of the biceps tendon can make you look like Popeye after he eats his spinach. In fact, eating spinach did not cause the Popeye deformity which I can take our muscles. This deformity may not look great, however it is not a serious injury, it does not need surgery, and the patient does not lose the use of the arm. Spinach is a healthy super food! Strong biceps is a symbol of healthy muscle strength. Here is a story of an ordinary person who picked up a heavy weight with his hand and then felt a sharp pain in his shoulder suddenly. The biceps move in his upper arm and he developed a baseball sized bulge, and his arm looks bigger, especially when he flexing the biceps muscle. Initially the muscle was painful, but the pain improved with 10 weeks. This deformity is called a Popeye deformity due to the rupture of the long head of the biceps at the shoulder area. When he picked up the heavy object, the biceps tendon ruptured at the shoulder. The bicep muscle goes from the shoulder to the elbow. It stays anchored in its position by 2 tendons that keep the springlike muscles stretched from the shoulder to the elbow. When it ruptures at the shoulder, the muscle becomes free and shortens and becomes a big ball going towards the elbow area. This usually occurs in people over the age of 50 years old. The tendon can become weakened by inflammation, by wear and tear, and this is called biceps tendinitis. We usually do not treat the Popeye muscle (do not do surgery). We give the patient anti-inflammatory medication and leave it. Some older guys like this deformity! They come to the clinic flexing their muscles. In some cases of persistent shoulder pain, the surgeon will create this deformity by cutting the tendon surgically with the intention of relieving the shoulder pain. This procedure is called biceps tenotomy. Subjective cramping can be a problem after this procedure (almost like a charley horse type pain in the arm). What is the function of the biceps muscle? The biceps flexes the elbow and it is a very powerful supinator of the forearm. When it ruptures at the elbow it should be repaired, otherwise the patient will lose about 40% of supination of the forearm.
