The problem is that Mars is seldom close to Earth. It’s only a reasonable distance about every two years as our planets spin around the Sun. What do you do if some Martian gets a toothache? How about a swollen or ruptured appendix? Sure, you would send a surgeon and a dentist along on the trip, but you’d need at least two in case a dentist needed a dentist, or the doctor needed a surgeon, right? But if one doctor or dentist died, the other would have no recourse and the entire colony might become dependent on keeping that one person alive more than any other.
